Employment Benefits


Personal Time Off:

After having completed six (6) calendar months of employment (Probationary Period), each regular full-time employee and regular *part-time employee of CUsource shall use accrued PTO according to the following schedule:

     1st year                                       13.33 Hours per month     20 days
     2nd through 4th year                  14.67 Hours per month     22 days
     5th through 9th year                  16.67 Hours per month     25 days
     10th year through 15th year       20 Hours per month          30 days
     16th year and beyond                 23.33 Hours per month     35 days


* Employees regularly scheduled to work less than 40 hours per week will accrue personal leave on a prorated basis.

Health/Vision/Dental Insurance:


CUsource provides a health insurance plan for employees that work at least 30 scheduled hours per week. The coverage is for the employee only and commences 90 days after employment. Employee is responsible for 20% of the monthly premium.

Aflac Insurance:


CUsource also allows Aflac insurance to be offered to all employees at their own cost. The premiums will be deducted from the employee's paycheck.

Flexible Spending:


All employees are eligible to participate in the flexible spending program upon after 90 days of employment. Open enrollment is the first day of each month.

Long-Term Disability:


CUsource provides an insurance program available for long-term disability for employees that work at least 30 scheduled hours per week. Employee coverage commences 90 days after employment. Coverage will start after 60 days of disability and is paid at the rate of two-thirds of an employee's salary.

Group Life:


CUsource carries a Group Life Insurance plan for employees that work at least 30 scheduled hours per week. The coverage is one time the employee's salary. Employee coverage commences 90 days after employment.

Retirement Plan:


CUsource provides a 401K retirement plan for employees that work at least twelve (12) continuous months from date of hire and completes 1,000 hours or more of service each year. An employee must also have reached the age of 21 years. Entry dates are quarterly. CUsource will deposit 2% or your monthly gross salary into a 401K monthly and will match an additional 5%.

Holidays:


CUsource observes the holiday schedule adopted by the 9th Federal Reserve District. Regular full-time employees are eligible for holiday pay.

Selection of Employees:


Employment with CUsource shall be open to any person who is qualified, and applications for employment shall be considered without regard to age, sex, religion, political philosophy, national origin, marital status, or irrelevant handicap. Currently employed staff shall be given consideration when a vacancy or a new position exists.
